On 30th September 2012 George Falder was found dead inside an industrial autoclave.
The fatal accident happened in tyre manufacturer Pirelli’s factory in Carlisle on Dalston Road.
Mr Falder was trapped inside a five-metre long machine for one of its cycles. He couldn’t get out because there was no way the autoclave could be opened from the inside.
An autoclave is a pressure-cooker like machine. It was used to heat up rubber tyre beads to temperatures of around 145 degrees at the factory in Carlisle.
Sometimes employees of Pirelli would enter the autoclave to pick up fallen beads in between the machine’s cycles.
George Falder was exposed to a deadly atmosphere for humans with barely any oxygen. This was created by the steam, which gets piped into the autoclave when turned on.
Pirelli Tyres Ltd pleaded guilty to a breach of the Health and Safety at Work Act. They were fined £150,000 and had to pay £46,706 for prosecution costs. This can sadly never make up for the loss of life and repair the devastating impact this fatal accident has had on Falder’s family and friends.
The Health and Safety Inspector Michael Griffiths said, “Pirelli failed to identify the risk posed by workers entering the autoclave. They should either have prevented access, or made sure that the autoclave was properly checked each time before the door was closed and the operating cycle started.”
This fatal work accident could have been avoided if Pirelli had protected their workers by controlling the risk that the industrial autoclave held. A sign or instructions might have been enough, but there were no preventative measures in place to stop Mr Falder’s colleague from switching the machine on with him inside.
Now, because the company didn’t have any system in place to stop this from happening they will have to live with this for the rest of their life. Employers have a duty to provide their employees with a work environment where risks to their health and safety are properly controlled. Pirelli Tyres Ltd failed on this front and George Falder paid the ultimate price.
