Vladislavs Golovacs sustained this life-changing injury as he attempted to clear a blockage for waste management and recycling firm Pinden at its quarry in Longfield, Kent, in December 2010.
Mr Golovacs had been instructed to remove stones from the driving roller of a conveyor belt at the premises, as they were causing the machine to vibrate and not work properly.
However, the equipment was not shut down before the 45-year-old began this task and his left arm subsequently became trapped in the moving mechanism, leading to the majority of the limb being ripped off.
Surgeons were unable to re-attach the arm and the professional has not been able to return to work since.
Andrew McGill of the Health and Safety Executive commented: "This was a horrific incident that was entirely preventable had appropriate guarding been fitted."
